MHC: David Brody, Nusrat Chaudry, Gabriella Sullivan
WCHC: Andrea Sandler, Sara Levin, Lynn Stromberg
Dr. Lyn Stromberg is also trained and part of the program and is available for home visits for patients at high risk of CHF readmission.
These providers will have one slot per clinic dedicated to addressing a patient in need of urgent follow-up to address CHF management and treatment issues. We hope to provide rapid and continual access, which is essential to stabilizing these patients. The goal is that these physicians be short-term consultants to primary care providers and their patients with CHF. The focus of visits will be to guide optimal medication management for these patients and to enhance patient education plus self-management of CHF.
Once the initial referral is made, the STRONG CHF providers will continue to follow the patient at a frequency appropriate to the patient's needs until the patient is on a stabilized regimen and can be discharged back to the sole care of his/her PCP. Please note that these visits will not supplant PCP visits for comprehensive care of the patient, and the CHF patient will remain with their PCP.
We are now accepting patients into these appointments. Referrals can be made directly from the PCP in clinic; just ask your nurse to schedule into the next CHF STRONG provider slot. At the time of hospital or ED discharge, the CHF nurses below or the appointment unit can also schedule appointments with a CHF STRONG provider. Please also feel free to call your local CHF strong provider with any questions, concerns or suggestions as we start this new venture.
